On some of our Sites, we offer health assessment tools that ask you to provide self-assessment information. Media Release
I hereby agree and give my permission for The Canadian Academy for Dawah Arts and/or partners and representatives to record, film, photograph, audiotape or videotape my or my child’s name, image, work, and performance (hereinafter collectively referred to as “Works”) and to display, publish or distribute these Works for the purpose of publishing, posting on The Canadian Academy for Dawah Arts website, posting on social media sites and/or for broadcasting on television or radio as determined by The Canadian Academy for Dawah Arts. I hereby waive any right to approve the use of these Works now or in the future, whether the use is known to me or unknown, and I waive any right to any royalties related to the use of these Works.
I understand that the Works may appear in electronic form on the internet or in other publications outside of The Canadian Academy for Dawah Arts’s control. I agree that I will not hold the The Canadian Academy for Dawah Arts responsible for any harm that may arise from such unauthorized reproduction. I have read this Media Release Consent Form and I fully understand the contents and meaning of this release. Health and Safety Waiver
I grant the staff members and employees of The Canadian Academy for Dawah Arts to act in loco parents (in lieu of the parent or guardian) during their programs. I agree that the staff member or employee of The Canadian Academy for Dawah Arts can administer basic medications or drugs in either prescription form or otherwise to my child, take him/her to a medical practitioner for medical treatment and approve medical procedures in the case of an emergency, which is to be determined solely by the staff member or employee of The Canadian Academy for Dawah Arts. I agree to pay for all such medical expenses.
I take full responsibility to inform The Canadian Academy for Dawah Arts in writing regarding participant’s medical history, including all drug and food allergies, pre-existing illnesses, physical, behavioral, emotional concerns.
The Canadian Academy for Dawah Arts has the full right to reject my child(s) from any course or activity if they cannot accommodate my child(s) special need or medical condition.
I acknowledge that I have fully read, understood and unconditionally agree to all the provisions outlined above.
Policies and Procedures
PROGRAM CANCELLATION & DE-REGISTRATION
All Courses and Camps are subject to sufficient registration. The Canadian Academy for Dawah Arts reserves the right to cancel or combine individual Courses or Camps if minimum registration is not achieved. A full refund will be issued in the event of a cancellation.
The Canadian Academy for Dawah Arts aims to provide a safe, positive, and welcoming learning environment. The Canadian Academy for Dawah Arts reserves the right to refuse admittance to or de-register any person who prevents our ability to provide such an environment. In the event of de-registration, a refund will be issued for all unattended days.
SPECIAL NEEDS
All details of special needs must be disclosed at the time of registration to establish program fit.
Although The Canadian Academy for Dawah Arts would love to accommodate all special needs and will strive to do so, we reserve the right to reject the registration of any participant, if we cannot provide a safe educational environment for them.
A certificate issuance policy
The Canadian Academy for Dawah Arts has the right to award various degrees as it sees fit.
The Canadian Academy for Dawah Arts has the right to grant a certified trainer certificate to whomever it deems appropriate according to its conditions.
The Canadian Academy for Dawah Arts has the right to withdraw any certificate it granted and prevent its use whenever it wants and without referring to the persons.
The opinions, activities, etc., of the recipients of any document from the Canadian Academy for Dawah Arts, do not reflect the Academy’s policies and opinions.
The Canadian Academy for Dawah Arts does not bear any responsibility in the case of misuse of the certificate issued by it.
The Canadian Academy for Dawah Arts has the right to choose the appropriate method and way of awarding certificates.
The sums paid for obtaining certificates of all kinds are not refunded, nor if the test designated for some certificates is not passed.
Those who hold a certified trainer certificate from the Canadian Academy for Dawah Arts are not entitled to grant this certificate to any other person.
